What to Bring to Your Examination

  • Medical and Vision Insurance Cards
    • Many medical insurance plans cover routine vision exams when you are diagnosed with certain health problems
    • Bring along your Prescription Drug card. We will try our best to choose medications that are covered by your insurance if they are needed.
  • NEW PATIENTS WHO WEAR CONTACT LENSES
    • Please bring your contact lenses, contact lens case, and the boxes or foiled packets that your lenses came in. If you happen to have a written prescription for your contacts, you can bring that as well. This helps us identify the type of contact lenses that you are wearing, which is particularly helpful if you are not happy with your current contacts lenses. We wouldn’t want to fit you in the same thing!!
  • Current Eyeglasses, Prescription Sunglasses, Reading Glasses, or Computer Glasses
    • If you are wearing an older pair of glasses because your current glasses aren’t working, please bring these to the exam as well.
  • List of all eye drops and oral medications that you take on a regular basis
  • Contact information for your medical doctor as well as a list of your current and past health problems
